Remarks: Observed an employee wash hands without using soap. Observed an employee wash hands in the mop sink. Observed 6 pans of cooked beans cooling in the walk in cooler over 6 hours with temperatures above 41F. Observed employee use gloved hands to handle raw meat and proceed to wash the gloves.

Remarks: Caesar dressing made in-house contains raw, shelled, unpasteurized egg as per the person in charge. Shellfish (clams & mussels) in prep cooler removed from the original container. Mussels were DISCARDED because the tag could not be located. Single use items (trays) not covered in storage room. Gasket torn on prep cooler. Floors in walk in cooler with build up. Shelving in walk in cooler with heavy build up.

Remarks: Items in the sliding door cooler such as ham opened and held for over 24 hours was not date marked at the time of inspection. Lemons for tea on display for self service in container with lid, no sneeze guards or other protection present. Several boxes of chips stored directly on the floor of the food storage area. Two employees wearing bracelets while working with food.
