Remarks: Observed an employee wash hands without using soap. Observed an employee wash hands in the mop sink. Observed 6 pans of cooked beans cooling in the walk in cooler over 6 hours with temperatures above 41F. Observed employee use gloved hands to handle raw meat and proceed to wash the gloves.

Mussels were DISCARDED because the tag could not be located. Single use items (trays) not covered in storage room. Gasket torn on prep cooler. Floors in walk in cooler with build up. Shelving in walk in cooler with heavy build up.
