Remarks: Several food items were not being maintained at 41 F or lower. White bean and ham soup was not being properly hot held. Foods held under time as a public health control must have starting temperatures of 41 F or lower for cold foods AND 135 F or higher for hot food items. Each food item must be labeled to indicate the start and discard time. 2ND CONSECUTIVE VIOLATION OF CODE SECTION. Foods not discarded by the appropriate dates. Employee did not change gloves between handling dirty dishes and clean dishes. 3-door cooler (contains house salad dressing and other TCS foods for salad) was malfunctioning.

Remarks: Multiple potentially hazardous foods not held at 41 degrees F or below and no time controls in place. Three chemical spray bottles did not have the common name of the chemical on them. Multiple food contact surfaces being sanitized in a 200 ppm chlorine sanitizer.
